Session 6 Times of Transition

This session gives information and strategies on dealing with the period after diagnosis, starting school and high school, finishing school and transitioning to independent living, if applicable. It looks at issues associated with bringing up a family whilst living with an NMD as well as older age. It also addresses issues associated with end-of-life planning and palliative care.

At the end of this session:

  • You will be able to identify issues facing children and families living with an NMD at times of transition, including starting school and starting high school.
  • You will be able to recall the issues and strategies for young people finishing high school, including transition to work, further study, community participation and independent living.
  • You will be able to outline the important differences between the paediatric and adult health systems and how these changes can be planned for.
  • You will be able to identify some issues and strategies for older people with an NMD and have some knowledge about the transition to aged care, if applicable.
  • You will also be able to recall some methods for planning for end-of-life and palliative care if this is applicable to your client.
